Implementation of PROM/PREM in Primary Care for Elderly Patients
Description:
The project aims to integrate Patient-Reported Outcome Measures (PROM) and Patient-Reported Experience Measures (PREM) into the primary care network for elderly patients. This collection of patient experiences and needs data is shared with healthcare providers and insurance companies to support targeted care interventions. Additionally, the project promotes the development of sustainable contracting models utilizing PROM/PREM data.
Primary Users:
- Patients: To receive holistic, patient-centered, high-value care that recognizes and addresses the whole human being - emotional, social and physical - beyond mere symptom management.
- Primary care physicians: To do justice to patients’ holistic needs and identify important, hidden health problems.
- Physician networks: To increase the trust of the various stakeholders in doctors' networks (healthcare professionals, health insurers, public authorities, politicians) and to make life easier for GPs in the network.
- Health insurances: Discover ways to improve the value of care through jointly defined contractual measures with physician networks and promoting VBHC/PROM mindset within the Swiss healthcare system, particularly in primary care.
Functionality:
Patients regularly provide feedback on their health and treatment experiences by ePROMs. This data is securely collected and made accessible to relevant healthcare institutions. The results are used to optimize patient care.
Objective:
To find out if PREMs / PROMs are effective tools to improve value in primary care in Switzerland.
